- Premium costs split between employer and employee

Dental Plan: Coverage begins the 1st day of month following 30 days post hire date*
Delta Dental Plan of TN - Base Plan
- Employee Funded Plan
- Calendar year benefit is $1,000
- Routine paid 100%
- Basic paid 80%
- Major paid 50%
- Orthodontics paid 50% up to a lifetime maximum of $1,000
Delta Dental Plan of TN - Buy Up Plan
· Employee Funded Plan
· Calendar year benefits is $1,500
· Routine is paid at $100%
· Basic is paid at 90%
· Major Services is paid at 60%
· Orthodontics is paid 50% up to a lifetime maximum of $1,000
Vision Plan: Coverage begins 1st day of month following 30 days post hire date*
Guardian VSP Vision:
- Employee Funded Plan
- Vision Examination: $10 co-pay
- Materials: $25 co-pay in plan materials
- Contacts: $25 co-pay if medically necessary
Retirement: Participation at the first quarterly enrollment period after 90 days of employment and if 21 years old
401(k) Principal Financial Group
- Employee is fully vested in any funds contributed by Remuda Ranch after five years employment.
Life/Accidental Death and Dismemberment/Disability*
UNUM Life/AD&D
- Remuda provides at no cost to employee.
- Effective the 1st day of the month after 90 days employment.
- Benefit amount is annual salary up to a max of $50K
UNUM Basic Long Term Disability
- At no cost to employee, Remuda Ranch provides a 40% Long Term Disability effective the 1st day of the month after 90 days employment.
- Employees have the option to buy up their Long Term Disability to 60% at their own cost.
UNUM Voluntary Short Term Disability
- Employee Funded Plan
- Employee can enroll in this plan the 1st day of the month after 90 days of employment.
UNUM Supplemental Life/AD&D
- Employee can purchase supplemental Life and AD&D.
Principal Financial Group - Flexible Spending Account - Pretax
- Can be used for a variety of approved applications including medical expenses not covered by the insurance and for day care.
